1. What is the Comparis Efficiency Award?
Comparis has been awarding its Efficiency Award since 2015. The aim of the award is to highlight the efforts made by health insurers to make their administration more efficient. Swiss health insurers with the lowest administrative costs receive the award.
Here you can find the entire Comparis analysis of administration costs as a PDF.
In the Comparis ranking, a health insurer is considered efficient if it is among the top quarter of health insurers with the lowest administrative costs. The administrative costs per insured person are assessed in francs.
For 2025, the efficiency threshold is 166.50 francs per insured person. The figures are based on the latest official supervisory data for mandatory health insurance (OKP).
2. How high are the administration costs of health insurers in Switzerland?
In 2025, health insurers further reduced their administration costs. On average, they still amount to 4.2% of premiums, or 191.15 francs per insured person. In 2024, administration costs were 4.5% or 193.00 francs.
Administrative costs show how much a health insurer spends on its administration and operation. These costs include:
Personnel costs
IT
Premises
Advertising
Healthcare services are not part of administrative costs.
3. How big are the differences between the health insurers?
The most efficient health insurer in 2025 is the Sodalis Gesundheitsgruppe. It has reduced its administrative costs by 25.45 francs compared to 2024, to 103.35 francs. This corresponds to 3.3% of the income from basic insurance premiums for this health insurer.
On highest are the administrative costs at Glarner Krankenversicherung cooperative. It spends 445.20 francs per insured person on administration. That is 150.15 francs more than in 2024.
Administrative costs only account for a small proportion of the premium. But they are among the costs that health insurers themselves can directly influence. Those who keep their administration lean over the long term use premium money efficiently. However, it is crucial that service and claims processing do not suffer as a result.
Are you dissatisfied with your health insurer?
You can switch basic insurance until 30 November.
Important: The Termination must reach the health insurer by the last working day of November at the latest. If 30 November falls on a weekend, it is the Friday before.
Size does not in itself guarantee efficient administration. High administration costs are not bad per se. However, those who spend significantly more than the most efficient insurers should be able to explain what additional benefits policyholders receive in return.
4. Several health insurers cross the 5’000-franc threshold
With several health insurers, policyholders will on average pay over 5’000 francs in 2025:
The highest is the average annual premium at Vivacare: There, policyholders paid 5’312.05 francs In terms of efficiency, Vivacare ranks 22nd out of 36.
Policyholders also have to pay over 5’000 francs with Avenir: Here, the average premium in 2025 is 5’147.90 francs. With management costs of CHF 202.20, the health insurer is among the more efficient half of health insurers.

7. Changes in administration costs since 2021
Between 2021 and 2025, administration costs per person fell on average by 3.3% In 2021, they were still 197.65 francs, and in 2025, they were 191.15 francs.
The fact that administration costs have been falling since 2021 is a positive development. It shows that efficiency gains are possible. But we shouldn’t be fooled by this: more efficient administration alone will not stop premium increases. The far greater leverage lies in the costs of medical services.
