If you only need basic health insurance, you are not required to fill out an application form.
When applying for supplementary health insurance, you must pay attention to the following:
Answer all questions about your state of health carefully and truthfully. If any questions arise, ask your doctor or current health insurer. Rather state one health problem too many than too few.
If an insurance rep fills out the form for you, make sure all information is correct and complete. All questions are equally important even if the representative may not make them out to be.
Add the following or a similar remark to your application: "I have answered all questions about my health as truthfully and completely as possible as a non-professional and as far as my memory goes back. However, I cannot guarantee that all information about my health is included and completely correct."
Inform the insurer by registered letter if your state of health changes or you are diagnosed with an ailment during the time span between having sent your application and closing a contract.
